NCCARE360 Shares Inaugural Quarterly Report

The inaugural Quarterly Report from NCCARE360 is now available. This report, designed to be the first in a series of reports, provides an overview of NCCARE360 and its components, and details the development and rollout efforts as of March 31,2019.

NCCARE360 is the first statewide coordinated care network that will serve as the core infrastructure as North Carolina moves toward whole person health and health system transformation. NCCARE360 provides users with public access to local health and social resources, access to dedicated care navigators, the ability to send and receive secure electronic referrals, communicate in real-time, securely share client information, and track outcomes.

NCCARE360 is the result of a strong public-private partnership between the NC Department of Health and Human Services and the Foundation for Health Leadership & Innovation and is being built in collaboration with United Way of NC, NC 2-1-1, Expound Decision Systems, and Unite Us.
