NCCARE360 is the first statewide network that unites healthcare and human services organizations with a shared technology that enables a coordinated, community-oriented, person-centered approach to delivering healthcare in North Carolina.
Access to high-quality medical care is crucial to health, but research shows that up to 80 percent of a person’s health is driven by social, economic, and environmental factors and the behaviors that emerge as a result. These drivers are often referred to as “social determinants of health” and includes things like housing, food, education, employment, transportation, and safety.
Currently, health care and social needs providers are not well connected and community-based organizations that provide only limited types of assistance are not well connected to other organizations who could help meet the holistic needs of a person. People needing multiple services often have to try and navigate across many organizations, a process can be both time consuming and inefficient.
NCCARE360 solves these problems by connecting organizations (such as clinicians, social workers, care coordinators, families, and other community-based organizations) through a shared technology platform that enables providers to send and receive secure electronic referrals, seamlessly communicate in real-time, securely share client information and track outcomes.
NCCARE360 offers a robust, integrated resource database, website, call center, and care coordination platform to connect people to the community resources they need, ultimately save time and money, helping all North Carolinians receive the care they need beyond just addressing disease.
NCCARE360 is a vision, not just a technology tool. The vision is to:
- Build a whole-person-focused system of health that helps people access the services and resources they need.
- Help people connect with others to improve the delivery of health care, increasing the overall efficiency of the system.
- Provide visibility and accountability bridging the gap between health care and social services.
NCCARE360 is the first statewide coordinated care network that includes a robust data repository of shared resources and connects healthcare and human services providers together to collectively provide the opportunity for health to all North Carolinians.
NCCARE360 is the result of a public/private partnership between the Foundation of Health Leadership & Innovation and the North Carolina Dept. of Health and Human Services, with implementation services provided by the following partner organizations:
- United Way of North Carolina and its NC 2-1-1 information and referral system
- Unite Us, the nation’s leading care coordination platform integrating healthcare and social services
- Expound Decision Systems, a leader in business intelligence, system integrations, and data repositories to develop NCCARE360 and make the vision a reality
Currently, the partners are working on a statewide implementation. NCCARE360 receives support from a broad array of public-private partners, such as Blue Cross and Blue Shield of North Carolina Foundation, Aetna Foundation, UnitedHealth Group, Gateway Health, the North Carolina Medical Society, the North Carolina Health Care Association, the North Carolina Community Health Center Association, the North Carolina Academy of Family Physicians, the North Carolina Pediatric Society, Community Care of North Carolina, the North Carolina Association of Local Health Directors, the North Carolina Association of Free & Charitable Clinics, No Kid Hungry NC (an initiative of the UNC Center for Health Promotion and Disease Prevention), the North Carolina Coalition to End Homelessness, the North Carolina Coalition Against Domestic Violence, NC Alliance of YMCAs and the North Carolina Department of Health and Human Services.
